As the parent/guardian of the minor named above, I give permission for my child to participate in Trinity Kidz Club regular meetings and special activities from August 13, 2025 through May 31, 2026.

In the event of a medical emergency, I authorize necessary treatment by a licensed medical professional if reasonable efforts to contact me have been unsuccessful.

I understand that participation involves normal risks and agree, to the extent permitted by law, not to hold the church, its staff, or volunteers liable for accidental injury or loss of personal belongings during Kidz Club activities.

Photos and videos may be taken during Trinity Kidz Club activities (including but are not limited to) church and ministry purposes, presentations, church website, social media, and promotional materials.

By granting permission, I authorize the church to use photos or videos of the minor named above for these purposes.
